**About us**:
ttainable Care is a leading Registered NDIS and TAC Provider dedicated to creating a world of possibilities for individuals with disabilities. We are dedicated to providing a range of tailored services, including Supported Independent Living (SIL) and Specialist Disability Accommodation (SDA) homes. We are currently seeking a passionate and committed Support Coordinator to join our growing team at Mount Waverley. Please note, this is an office-based role that requires regular travel to our Mount Waverley office.

**About the position**:

- Accept and follow up referrals for Support Coordination
- Empower participants to understand, implement and review their NDIS plans
- Support the individual’s ability to connect with informal, mainstream, and funded supports
- Assess and obtain relevant NDIS documentation with participants during intake and in the lead up to plan reviews
- Monitor plan expenditure and support participants to remain informed about their expenses
- Actively maintain professional knowledge in the areas of disability, working within the NDIS guidelines
- Ensuring the quality of support is in line with best practice and in line with allied health, Support Coordinator / Case Manager recommendations
- Provide crisis management and incident reporting follow up
- Comply with NDIS progress, incident, change of circumstance and other required reporting

**Qualifications & experience**
- Must have 2 years of experience in NDIS Support Coordination/Community Case Management Role
- Excellent written and verbal communication
- Self-starter and a team player that is willing to make a positive difference
- Ability to work collaboratively with anyone in our organization
- A driven individual with a passion for working within the disability and mental health industry
- Liaison and networking
- to maintain current knowledge and build contacts for client referrals
- Service provision NDIS to work with the client to better understand and implement their NDIS plan accordingly

**Mandatory Requirements**
- A current Working With Children’s Check is essential
- Have full Australian working rights and a National Police Clearance
- National Disability Insurance Scheme (NDIS) Check
- Driver Licence (preferred)

Pay: $70,000.00 - $85,000.00 per year

**Experience**:

- disability service: 1 year (required)
- NDIS support coordination: 1 year (required)

Licence/Certification:

- NDIS Worker Screening Check (required)
- Full Driver Licence (required)
