**The role**

As a Mentor, you will provide care, supervision and support for people with disabilities in the home, residential establishments, community and hospitals. You will also work with Support Coordinators and health professionals to maximise the individual's physical and mental wellbeing.

**Duties**
- Provide support to participants with physical, and/or intellectual, and/or psychosocial disabilities
- Understand and provide appropriate care to participants who demonstrate a range of challenging behaviors
- Assisting with activities relating to daily living, community support, and empowerment for the participant
- Providing a person-centered service, through choice and control
- Writing and submitting file notes at the end of each shift
- Follow direction and guidance from the supports in the participants life i.e. Support Coordinator, Behaviour Support Practitioner and the LSP Coordinator.

- Work within professional boundaries
- Ability to work effectively under pressure
- Excellent communicator and active listener

**Criteria**
- Minimum Cert IV Qualification in Community Services Sector (desirable)
- Working with Children Check
- Current Driver’s Licence and Vehicle
- NDIS Worker Orientation Module and New Worker - Induction Module
- Fully vaccinated for Covid-19, as directed by the Public Health Order.
