**About the Health Independence Program (HIP) department**
HIP encompasses a wide range of services that deliver healthcare to support the transition from hospital to home, and to help reduce unnecessary readmissions to hospital or presentations to the emergency department.

**About the role**
As a Central Intake and Fast Track Care Manager you will provide client assessments to work out what clients need to support discharge and assist them to remain safely at home. This is primarily an office based role. The position has a range of functions: assessing and managing client needs post discharge, monitoring client progress, arranging ongoing supports as needed and working in the central intake service triaging referrals to the various HIP services. This is a great role where you will use your great communication skills and broad health service knowledge.

A regular day will see you:

- Triaging referrals into the Austin HIP program, liaising with referrers, services and clients
- Assessing clients to ensure they are well supported at home post discharge
- Organising short term supports and referrals for clients who need ongoing services in the community.
- Enjoying the camaraderie that working in a highly skilled and energetic multidisciplinary team brings

As the Care Manager, you will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. You’ll bring energy and drive to your work, and have a keen interest in developing your skillset.

- Advanced communication skills - both oral and written
- Well developed holistic assessment skills
- Some knowledge of the community and aged care system
