**We have exciting **Permanent Full-time** Aboriginal Community Support Worker opportunity available in** **Bowral, NSW.**

These positions involve rostered shift work, and you will need to be available to work flexible shifts that may include a combination of **mornings, evenings and weekends.**

Based in **Bowral**, you will report to the Team Leader in our Department Team.

**Key responsibilities will include**
- Working with customers and focusing on their wellbeing
- Working face to face with customers in the community to develop wellbeing plans
- Providing support to customers to improve their independence by assisting with living skills, transport, social activities, medication and personal care, as well as other support as required
- Developing and maintain professional relationships and networks with customers, families, caregivers and key stakeholders
- Seek out innovative ways to enhance the customer journey
- Reconnecting with families and Indigenous communities
- Participating and engaging in activities and outings in the community and building relationships and connections in the community
- Keeping customer records up to date on a daily basis using on line systems by completing detailed notes and reports on customer progress and achievements
- Managing some challenging behaviours

**You'll also need**:

- Minimum Cert III in Mental Health, Disability, Community services, Aboriginal Studies (or relevant experience and willingness to obtain qualifications)
- Availability to work across a rotating roster
- Valid NSW Drivers C class licence, with ability to drive vehicles in licence class, including large cars and vans.
- Willingness to obtain an Australian Federal Police Check, WWCC and first aid certificate that meet New Horizons employment criteria prior to commencing with us
