The Melbourne Clinic (TMC) are the lookout for an experienced and passionate Outreach Program Manager to lead our outreach initiatives and expand our impact in the community. This role will be crucial in developing and managing programs that connect individuals with vital mental health services and resources.

The Melbourne Clinic (TMC) is a large private mental health service, comprising of inpatient, Day Program and Outreach services. You will work with the team to achieve operational goals and maintain clinical integrity of therapeutic programs.

**Position summary**

The Outreach Program Manager will oversee the efficient organisation, coordination, and competent operational management of the outreach services. This role involves leading a multidisciplinary team to ensure the delivery of high-quality care, from initial referrals through to assessment and ongoing support.

**As the Outreach Program Manager, your responsibilities will include**:

- Oversee the Outreach Clinical Continuum of Care including referral responses, assessment, care planning, care implementation, care review, and separation processes
- Lead and manage the performance of a designated multidisciplinary Outreach team
- Ensure service compliance with all relevant legislation and policies
- Facilitate effective communication with referrers, patients, carers, treating psychiatrists, inter-facing programs, and outreach staff
- Manage resources allocated to the Outreach service, including staff and vehicles

**Essential Criteria**:

- Appropriate tertiary qualifications in social work, clinical psychology, occupational therapy and nursing
- Eligibility for membership of the above professional Australian Associations
- 5 years’ experience working within the mental health space
- Minimum of 2 years’ managerial experience
- Strong leadership, communication, and organizational skills
- Proven ability to work collaboratively and build relationships in the community with ease
- Well-organised with strong time management skills
- Full valid Australian driver’s license
