Job Title: Care Manager

We are seeking a compassionate and dedicated individual to fill the role of Care Manager in our organization.

  • This position involves providing leadership and support to the clinical and personal care teams, ensuring high-quality care is provided to residents at all times.

Key Responsibilities:

  • Provide clinical leadership and coaching to the clinical and support teams to ensure delivery of quality resident outcomes.
  • Work closely with policies and procedures, including IPC, to ensure compliance with quality standards.
  • Assume second-in-charge responsibilities to the Residential Manager.

Requirements:

  • A kind heart and genuine passion for delivering exceptional care to the elderly.
  • Current AHPRA Registration as a Registered Nurse.
  • Thorough understanding of aged care legislation and quality standards.
  • Demonstrated knowledge of AN-ACC assessment processes.
  • Comprehensive clinical knowledge and understanding of aging process and age-related health issues.
  • Knowledge and understanding of workplace health and safety.
  • Previous experience working in residential aged care in similar role.
  • Strong coaching/training skills and ability to influence others.
  • Customer-centric approach to business.
  • Ability to work independently and collaboratively within multidisciplinary team.
  • Competent IT skills including full Microsoft suite.
  • Willingness to obtain Police Check and NDIS clearance.
