About The Role
Grafton Community & Allied Health have an exciting career opportunity for a Social Worker to join their Transitional Aged Care Program. This role provides high quality clinical services and leadership as an integral part of the multidisciplinary health care team for patients/clients of the Northern NSW Local Health District (NNSWLHD). A relocation package maybe available to the successful candidate.
