This role presents a powerful opportunity to collaborate with women experiencing homelessness and rough sleeping, helping them achieve a more secure future.
About the Program- We provide short-term case management and assertive outreach support to women over 20 without dependents, experiencing homelessness across Melbourne's inner-west.
- We offer up to 3 months of support to help clients access crisis accommodation, housing, health services, mental health support, and community connections.
- You will build strong relationships with clients using persistent engagement techniques.
- Develop individualized case plans aligned with client goals and immediate needs.
- Facilitate access to essential services, ensuring continuity of care.
- Work collaboratively with internal and external partners to ensure best possible outcomes.
- Evaluate and report on client progress, ensuring accurate documentation and timely reporting.
- Diploma or Degree in Social Work, Psychology, or a related field.
- Experience in case management and assertive outreach.
- Knowledge of homelessness systems and trauma-informed care.
- Strong communication and advocacy skills.
- Ability to work independently and collaboratively in fast-paced environments.
- Excellent communication and interpersonal skills.
- Strong analytical and problem-solving skills.
- Ability to work effectively in a team environment.
