Homeless Support Specialist

We are seeking a dedicated professional to join our team in North Melbourne and Flagstaff, VIC.
Key Responsibilities:- Build strong relationships and develop personalized case plans with clients experiencing homelessness.
- Provide outreach support and facilitate transitions into long-term housing solutions.
- Advocate for clients to access health, housing, legal, education, and well-being services.
- Collaborate with internal teams and external agencies to ensure coordinated care and support.
- Maintain accurate documentation and contribute to service quality and development initiatives.
- A degree in Social Work, Psychology, or related field.
- Experience in case management and supporting people experiencing homelessness.
- Knowledge of trauma-informed, strengths-based, and person-centered approaches.
- Excellent interpersonal, advocacy, and conflict resolution skills.
- Current Australian Driver's Licence and Employee Working with Children's Check.
