This role offers the chance to work with a team that provides essential support and acts as a single access point for advice, assessment, and responses to urgent matters.

The Team

">

As a key member of this team, you will be responsible for delivering high-quality client-centric case management practice. You will manage a caseload of complex clients and provide support and guidance to colleagues as needed.

Key Responsibilities:

  • ">
  • Manage clients with challenging behaviors">
  • Conduct assessments of client referrals where mental health issues are evident">
  • Record accurate case notes and other records in a case management system">
  • Negotiate client claim outcomes with relevant areas">
  • Conduct urgent risk assessments as required">
  • Provide advice and support to non-clinical staff undertaking assessments">
  • Undertake calls to clients to offer assistance or gather information">
  • Link clients to appropriate mental health services">
  • Occasionally provide short-term case management intervention">

Required Qualifications and Skills:

  • ">
  • Tertiary qualification in a relevant health field">
  • Registration with AHPRA or AASW is highly desirable">
  • 2+ years' experience working as a case manager or equivalent clinical role">
  • Extensive experience working in mental health or community case management settings">
  • Excellent written and oral communication skills">
  • Experience in triage, assessment, and case management via telephone">
  • Australian citizenship">

Desirable Expertise:

  • Experience working with military or veteran mental health services">
  • Expertise in supporting clients with trauma and substance abuse">
  • Understanding of Trauma Informed Care principles">

Assignment Details:
