We are seeking a highly skilled and compassionate nurse to join our team in delivering holistic care to older adults experiencing health challenges, chronic conditions, and complex needs. Our goal is to improve the lives of our clients by providing early intervention, health education, and recovery-focused support within their homes and community settings.

As a Community Nurse, you will play a vital role in delivering high-quality nursing care to our clients. This includes conducting clinical assessments, developing individualized care plans, and working collaboratively with other healthcare professionals to ensure comprehensive care.

Key Responsibilities
  • Providing direct clinical nursing care to clients in their homes, adhering to best practice nursing procedures
  • Conducting comprehensive clinical assessments, including wound care and medication management
  • Developing and implementing holistic care plans in collaboration with clients, carers, and multidisciplinary teams
  • Liaising with relevant parties to maintain accurate records and ensure seamless care transitions
Requirements
  • A current AHPRA registration as a Registered Nurse or similar qualification
  • Minimum 3 years postgraduate nursing experience, with a focus on community or primary health care
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders
  • Strong organizational and time-management skills, with the ability to prioritize tasks and manage multiple cases
