About the Role:
We are seeking a skilled and compassionate care specialist to join our team as a Community Nurse. In this rewarding role, you will provide holistic, person-centred care to older adults experiencing health challenges, chronic conditions, and complex needs.
Main Responsibilities:
- Provide direct clinical nursing care to clients in their homes in line with best practice nursing procedures
- Conduct clinical assessments, dressings, medication assessment, and catheter changing
- Develop a comprehensive nursing care plan in collaboration with client, carer, Care Advisor, and Allied Health where relevant
- Offer safe, quality nursing practice to clients with specific needs and/or complex needs, such as wound care, palliative care, education, and support
- Liaise with all parties involved in the care of the client to maintain holistic healthcare and wellbeing
- Ensure accurate and timely completion of all documentation
- Work safely to protect yourself, your colleagues, and clients by following all policies and procedures
Necessary Skills and Qualifications:
- Genuine passion for delivering high-quality care to the elderly
- Current AHPRA registration following completion of a Bachelor of Nursing or similar qualification
- Minimum 3 years post-graduate nursing experience
- Expertise in clinical assessments
- Ability to work independently with initiative
- Strong computer skills, especially Microsoft Office products, and ability to learn new systems
- NSW Drivers license and reliable vehicle
