About the Role

We are seeking a skilled Youth Case Manager to join our team. The successful candidate will support young people facing alcohol and other drug challenges, guiding them through recovery via effective case management and collaboration within a multidisciplinary team.

  • Deliver comprehensive case management support, including assessment, referral, and advocacy.
  • Maintain knowledge of current trends and best practices in substance abuse treatment.
  • Collaborate with external agencies for integrated care.
  • Support clients in strengthening family and community connections.
  • Maintain accurate records and develop case plans with clients and colleagues.
Responsibilities:
  1. To assess the needs of young people facing AOD challenges and provide guidance on recovery pathways.
  2. To deliver case management support, including assessment, referral, and advocacy.
  3. To maintain knowledge of current trends and best practices in substance abuse treatment.
  4. To collaborate with external agencies for integrated care.
  5. To support clients in strengthening family and community connections.
  6. To maintain accurate records and develop case plans with clients and colleagues.
Requirements:
  • A degree in Social Work, Youth Work, or related field, or a Diploma with relevant experience.
  • Experience in case management with complex or disadvantaged individuals.
  • Understanding of trauma, mental health, and youth development.
  • Experience working with diverse cultural backgrounds.
  • Current Driver's Licence, Working with Children Check, First Aid certificate.
