About the Role:
As a Community Care Coordinator specializing in Home Care Packages, you will play a pivotal role in helping seniors access the support they need to maintain their independence and lifestyle choices. Your primary responsibility will be to coordinate and deliver high-quality care services to clients in their homes, ensuring their needs are met and exceeded.
Key Responsibilities:
- Develop and implement personalized care plans for clients
- Coordinate with healthcare professionals and community services to ensure seamless care delivery
- Build and maintain strong relationships with clients, families, and healthcare teams
- Monitor and evaluate client progress, making adjustments to care plans as needed
- Collaborate with the care team to ensure effective communication and coordination
Requirements:
- Significant experience within the Aged Care industry in a similar role
- Understanding of the Aged Care Sector as well as Home Care Package and other program guidelines and service requirements
- The ability to build strong relationships and networks within the community
- Current Australian Driver's License
- Strong interpersonal and leadership skills
