We are seeking a dedicated and compassionate Care Support Specialist to join our team.
Job Summary:The Care Support Specialist will play a vital role in guiding individuals through their care journey, ensuring they receive high-quality services tailored to their unique needs and goals.
This dynamic role offers the opportunity to work closely with clients, helping them navigate their care options, develop tailored support plans, and access the services they need to thrive.
About the Role:The Care Support Specialist is responsible for supporting a defined portfolio of clients within various funding subsets, focusing on Community Care clients.
Key objectives of this role include:
- Provide information, support and referrals, completing intake processes and assessments as needed;
- Develop personalized care plans with clients, addressing their unique care needs, and outlining how services will support them achieve their goals;
- Ongoing monitoring of clients' progress against identified care goals, conducting reviews and assessments required to ensure full support;
- Collaborate with service coordinators to secure appropriate service bookings aligning with clients' identified goals;
- Oversee client budgets to ensure services are delivered in compliance with legislative guidelines;
The Care Support Specialist ensures all services are provided in accordance with the standard of care as per relevant funding bodies and program manuals.
This position may be required to assist other funding types when staff coverage is necessary.
Required Skills and Qualifications:Minimum two years' experience in care management, working within Disability, Aged Care or broader Community Care sectors;
Demonstrated experience supporting individuals through care goal development and personalized care planning; and
Current Driver's License (QLD).
Formal training in Case Management from a recognized course/program.
