**Care Support Specialist Position Available**

This is a permanent, full-time role offering coordination and transition support services for clients under the NDIS.

**Key Responsibilities:**

- Conduct needs assessments to develop tailored support programs.
- Collaborate with client representatives to onboard new clients, respecting individual choices and preferences.
- Create effective rosters of client services based on individual goals and expectations.
- Work closely with support workers and contractors to deliver high-quality service aligned with program objectives and expectations.
- Monitor and report on client progress towards established goals.
- Maintain regular contact to foster strong relationships with clients, carers, families, and other stakeholders involved.
- Design and implement socially inclusive programs and projects that enhance client well-being.
- Ensure accurate records and documentation are maintained in accordance with organizational and legislative requirements.
- Facilitate participation of service users in community activities and events.

**Required Skills and Qualifications:**

- 2+ years' experience as a Care Coordinator (preferred)
- Masters Degree (desired)
- Strong communication and interpersonal skills
- Ability to work independently and collaboratively as part of a team

- Competitive salary range: $75,000 - $80,000 per year

**Education and Experience:**

- Preference given to candidates with a Masters Degree and 2+ years of Care Coordinator experience

**Language and Licence/Certification:**

- English language proficiency (required)
- Driver Licence (desired)
- First Aid & CPR ,NDIS WSC and WWCC (preferred)

**Work Authorisation and Location:**

- Work permit for Australia (required)
- In-person work location
