**Job Overview**

We are seeking a Case Manager to provide alcohol and other drug counselling services to individuals in need.

This is a casual position based in Eagle Heights, QLD. The successful applicant will have the opportunity to work with a diverse range of clients and contribute to the development of case management plans and strategies.

**Key Responsibilities**
  • Facilitate assessment, intake, counselling, referral, educational/recreational support and case management activities for clients/residents of the centre
  • Develop and monitor individual treatment plans and maintain case management documentation, records, reports and data collection
  • Aquire and maintain a good working knowledge of AOD referral and support options for residents within our network and external agencies and work collaboratively and professionally with other agencies
  • Participate in team activities and attend staff meetings when scheduled and demonstrates a supportive approach to other staff members which contributes to the overall success for the service and clients
**Required Skills and Qualifications**
  • Tertiary or other qualifications within Community Services, Social Work or similar discipline, or extensive relevant experience with a minimum of Certificate IV qualifications in AOD
  • Current Driver's license
  • Maintain an Alcohol, Drug and Gambling free presence at all times
  • Solid level of experience (minimum 2+ years) in the implementation of case management and systems for disadvantaged and/or complex needs clients
  • Knowledge of or experience in the drug and alcohol service provision
  • Experience working with people from culturally and linguistically diverse backgrounds and religions
  • Experience working in collaboration with other organisations to develop and tailor programs to meet clients' needs
