Job Title: Neurodegenerative Care SpecialistRole Overview

As a Neurodegenerative Care Specialist, you will provide complex specialist nursing care and lead the development of specialist clinical practice and service delivery to ensure the provision of person-centred, specialised, multidisciplinary care for people living with neurodegenerative conditions, including Dementia, Parkinson's Disease, Multiple Sclerosis, Muscular Dystrophy, Motor Neurone Disease, and Huntington's Disease.

This role will focus on care at home in the community setting. You will use various modalities, including face-to-face, telephone, and telehealth, to deliver high-quality patient care.

The successful candidate will have experience in providing care to patients with complex needs and be able to work effectively as part of a multidisciplinary team to achieve excellent patient outcomes.

Key Responsibilities
