Youth Wellness Journey Advocate

This is a challenging and rewarding role that provides support and guidance to families as they navigate their youth's wellness journey. Working collaboratively with caregivers, youth, and other stakeholders, the Family Support Worker plays a key role in achieving established goals and objectives.
Duties and Responsibilities:- Embracing a holistic approach to youth well-being, informed by the Pan-Canadian Guiding Principles of Integrated Youth Services and the social determinants of health.
- Providing direct service to caregivers and families under the clinical supervision of the site Mental Health Administrator clinician.
- Meeting with caregivers to assess their needs and options, and providing information, education, and ongoing support and problem-solving as needed.
- Facilitating referrals to services both internal and external to Integrated Youth Services, supporting caregivers and youth to create and maintain connections to relevant services/care providers, and monitoring their progress.
- Assisting caregivers to support young people on their wellness journey and in the successful transition to adult services.
- Assisting with case management and crisis intervention.
- Collaborating with Integrated Youth Services colleagues and others to coordinate and integrate service delivery at all levels.
- Leading the development and implementation of caregiver and youth peer support initiatives for the site.
- Planning and facilitating group programming and activities for families/caregivers, including peer support, capacity-building, and other relevant programs.
- In collaboration with the provincial Youth and Caregiver Engagement Coordinator, organizing and supporting the site-based Caregiver Advisory Committee.
- Actively nurturing and supporting formal and informal opportunities for caregiver engagement, peer support, and leadership.
- Bachelor's degree in a social science required; Family Studies or similar field preferred, with specialization in mental health and addictions an asset.
- Two years of relevant experience required; three to five years preferred, with experience providing services directly to parents/caregivers and working with adolescents.
- Current and up-to-date CPR and First Aid training, Naloxone training, Non-Violent Crisis Intervention training, Brain Story training, and Mental Health First Aid training required.
- Valid driver's license, access to a reliable vehicle, and appropriate insurance to transport clients.
- Compassion, empathy, diplomacy, and problem-solving skills, with the ability to work independently, as part of a team, and with guidance.
- Exceptional attention to detail and verbal and written communication skills.
- Ability to work effectively with others toward shared goals, with a high level of integrity, confidentiality, and accountability.
- Travel may be required.
- Manual dexterity required to use desktop computer and peripherals.
- Overtime may be required; working evenings/weekends as needed.
- Interacts with employees, management, and the public at large.
- Environment can be busy and/or noisy at times.
