Job SummaryWe are seeking an experienced and dedicated Case Manager to join our team. This role involves providing leadership to a team of residential care workers, developing and managing individualized plans for young people, and working collaboratively with other departments and stakeholders to ensure a cohesive approach to youth support.
Key Responsibilities- Provide leadership and guidance to a team of residential care workers, including conducting regular supervisions and fostering a supportive team environment.
- Develop, manage, and review individualized plans for young people, addressing their specific needs and goals.
- Collaborate with other departments, stakeholders, government agencies, and community partners to ensure a cohesive approach to youth support.
- Undertake necessary mandatory reporting and other administrative tasks to ensure compliance with organizational policies and regulatory requirements.
- Attend and participate in regular supervision, team meetings, stakeholder meetings, and inter-agency meetings.
- Provide direct care support by completing shifts as needed, ensuring continuity of care and support for the young people.
- Certificate IV, Diploma, or Bachelor in Child, Youth, and Family Intervention or a related field such as Social Work, Psychology, Education, etc.
- Proven experience in youth services management, including program oversight and staff supervision.
- Thorough understanding of Child Protection principles and experience working with government and non-government agencies.
- Strong leadership and decision-making skills with the ability to inspire and motivate a team, manage conflicts, and complaints.
- Excellent communication and interpersonal skills, with the ability to build relationships and collaborate effectively with diverse stakeholders.
