**Job Opportunity:**

 

We are seeking a skilled social worker to join our multidisciplinary allied health team, providing high-quality care to patients and families across the Central Coast.

The successful candidate will work collaboratively with colleagues to contribute to service development, support staff and student education, and ensure access to community services.

Your duties include:

  • Assessing and counselling patients to address their emotional needs.
  • Coordinating case planning to ensure effective care delivery.
  • Ensuring seamless access to community services for patients and their carers.

**Essential Requirements:**

  • Team player: You will collaborate effectively with colleagues to deliver exceptional social work services.
  • Compassionate: You will provide emotional support to patients and carers during challenging times.
  • Dedicated: You will contribute to service improvement initiatives and support staff and student education.

 

This role is part of a dynamic multidisciplinary team working within a community-based approach. Our team provides comprehensive care services across multiple sites on the Central Coast.

You will work closely with colleagues and senior managers to develop and deliver proactive social work services.

The successful candidate will be active in areas such as service development, evaluation, and teaching, contributing to the growth and success of the team.

This position requires collaboration with other social workers to ensure timely and effective service provision across all geographical sites.

Clinical duties will be performed across multiple sites within CCLHD.

There may be occasional weekend or public holiday shifts required.
