As a skilled and dedicated clinical pharmacist, you will play a vital role in delivering high-quality patient care and contributing to the success of our Clinical Services team.

About the Job

This is a rotational position that involves working across the pharmacy dispensary and inpatient wards of our campuses. Your primary responsibility will be to provide excellent clinical pharmacy services by supporting customers, patients, carers, and health professionals with medication information and safety practices.

  • Compile and document accurate best possible medication history for hospital inpatients
  • Reconcile and clinically review prescribed medications to ensure optimal treatment outcomes
  • Manage supply of inpatient and discharge medications efficiently
  • Verify prepared medications prior to delivery to wards and patients
  • Prepare discharge medication summaries and provide personalized counseling to patients and carers
  • Work collaboratively with pharmacists, technicians, and students to provide supervision and assistance as required
  • Liaise with external facilities and community healthcare providers to ensure seamless continuity of care
  • Provide timely and accurate medication-related information, advice, and support to the multidisciplinary team

About You

To succeed in this role, you must possess:

  • Bachelor of Pharmacy degree
  • APHRA registration to practice as a pharmacist (without restrictions)

  • Previous demonstrated experience in ward-based clinical pharmacy
  • Ability to work flexible shifts, including evenings, weekends, public holidays, and on-call (with training)
  • Willingness to work at all Mater Hospital locations, including South Brisbane, Redland, and Springfield
