As a RN Case Manager, you will be responsible for facilitating the development and implementation of individualized patient plans triggered by referrals from nursing, physician, interdisciplinary rounds, and other care team members.

You will collaborate with patients, families, physicians, and interdisciplinary teams to ensure that care is provided across the continuum from acute care through discharge planning.

This role requires taking a leadership position with the interdisciplinary team to achieve optimal clinical outcomes, coordination, and continuity of healthcare as patients transfer between different levels of care.

The ideal candidate will have two (2) years of recent experience in clinical, utilization management, home care, discharge planning, managed care, and/or case management that has provided a broad understanding of Case Management and Utilization Management concepts.

Candidates should possess strong clinical competence, knowledge of disease processes, treatment protocols, and pharmacological management. Effective communication skills are essential, including clear and sensitive written and oral communication with physicians, patients, families, healthcare professionals, and others.

Key responsibilities include interacting and collaborating with physicians and other healthcare professionals in a professional manner, organizing and coordinating work efforts, prioritizing tasks, and solving problems.

Requirements
