Role OverviewThe Registered Nurse Case Manager plays a pivotal role in delivering high-quality hospice care, ensuring compliance with industry standards and regulatory requirements.
Key Responsibilities- Conduct ongoing assessments and provide personalized nursing care to patients in their homes or healthcare facilities, guaranteeing accurate evaluations of patient needs and the development of comprehensive care plans.
- Adhere to current professional nursing standards and agency-approved protocols for hospice care.
- Prioritize quality plan and symptom management for optimal patient comfort.
- Collaborate with other healthcare providers to share changes in patient condition and address evolving needs.
- Evaluate patient and caregiver needs, providing education when necessary to enhance patient outcomes.
- Demonstrate unwavering commitment to exceptional customer service for internal and external stakeholders.
- Current, unencumbered licensure to practice as a Registered Nurse (RN) in the state of Ohio is required.
- A minimum of one year's experience in acute care or medical-surgical nursing is mandatory; hospice experience is preferred.
- Proficiency in Windows, Microsoft Office, and the internet is essential; familiarity with electronic medical records (EMR) is advantageous.
