Job Summary

This position requires a skilled Nurse Practitioner to provide high-quality patient care and lead interdisciplinary teams in delivering comprehensive healthcare services. The ideal candidate will have expertise in primary care, exceptional communication skills, and the ability to work effectively in a fast-paced environment.

Duties and Responsibilities
  • Provides clinical examination, diagnosis, and treatment of acute and chronic diseases, preventive care, and coordination of care for a panel of primary care patients.
  • Responsible for monitoring PACT teamlet performance to assess adherence to VA standards, assure quality and effectiveness of care, and meet VA performance measures.
  • Leads the team in assisting the patient advocate and administration in resolving patient concerns and inquiries.
  • Exercises medical decision-making and provides a broad range of diagnostic and therapeutic services for a specific group of patients.
  • Must have excellent communication skills and relations with staff and community and is able to elicit cooperation from others.
Requirements and Qualifications
  • A post-master's certificate or master's or doctoral degree as a Nurse Practitioner.
  • Active, current, full, and unrestricted APN license to practice as an NP in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
  • Basic Certification as an APN (NP) from one of the following: the American Nurses Credentialing Center (ANCC), American Academy of Nurse Practitioners Certification Board (AANPCB), or other nationally recognized certifying body in the area in which the Nurse Practitioner is academically prepared.
