Job Summary:

As a Registered Nurse (RN) Case Manager, you will facilitate the development, implementation, coordination, and evaluation of individualized patient plans. Collaborate with patients, families, physicians, and interdisciplinary teams to ensure seamless care from acute settings through the continuum. Take a leadership role in achieving optimal clinical outcomes, coordinating health care services, and promoting effective resource utilization.

Key Responsibilities:
  • Facilitate patient plan development, implementation, and evaluation
  • Collaborate with patients, families, physicians, and interdisciplinary teams
  • Ensure continuity of care across different levels of care
  • Maintain quality, cost-effective, and medically necessary care
Requirements:
  • Bachelor's of Science in Nursing (BSN)
  • Two years recent experience in clinical, utilization management, home care, discharge planning, managed care, and/or case management
  • Demonstrated clinical competence and ability to interact professionally with healthcare professionals
