About the Role:

As a Care Manager, your primary responsibility will be to plan, manage, coordinate, and review client goals. You will communicate effectively with team members and clients to develop care plans, formalise agreements with clients and their families regarding care services and costs, and accurately document interactions with clients, families, or stakeholders and share relevant information with team members.

About the Candidate:

  • Previous experience working with seniors or aged care is essential.
  • You must be highly efficient, organised, and able to work under pressure.
  • Strong written and verbal communication skills are required.
  • Administrative experience and proficiency in using computers and systems are necessary.
  • A valid Australian Driver's Licence is mandatory.
