Job Summary

We are seeking a skilled professional to fill the role of Aboriginal Palliative Care Coordinator. As an integral member of our team, you will play a crucial part in providing culturally safe and respectful end-of-life care for Aboriginal people.

About the Role

The successful candidate will be responsible for working collaboratively with clinicians, community-based service providers, and patient families to ensure that palliative care needs are met. You will act as a conduit for Aboriginal patients with palliative care needs in both hospital and community settings, ensuring that care coordination practices are culturally competent and safe.

This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a multidisciplinary team environment. You will also need to possess a deep understanding of issues affecting Aboriginal people in relation to grief and loss, as well as social and emotional wellbeing.

Responsibilities
  • Develop and implement culturally safe environments for Aboriginal patients and their families within a multidisciplinary team.
  • Collaborate with clinicians, community-based service providers, and patient families to ensure that palliative care needs are met.
  • Act as a conduit for Aboriginal patients with palliative care needs in both hospital and community settings.
  • Work across SLHD to integrate culturally safe practices in care coordination.
Required Skills and Qualifications
  • This is an Identified Aboriginal/Torres Strait Islander Position. Applicants must be of Aboriginal descent through parentage, identification as Aboriginal and acceptance in the community.
  • Understanding of issues affecting Aboriginal people in relation to grief and loss and Aboriginal social and emotional wellbeing.
  • Ability to lead the development of culturally safe environments for Aboriginal patients and their families within a multidisciplinary team.
  • Holds a current NSW Driver's Licence (P1 and P2 acceptable).
