Job Description

We are seeking a skilled and compassionate Aboriginal Palliative Care Coordinator to join our team. This is an exciting opportunity for a dedicated professional to make a positive impact on the lives of Aboriginal people and their families.

The successful candidate will work collaboratively with clinicians, community-based service providers, and patient families to ensure respectful and culturally appropriate end-of-life care. They will act as a conduit for Aboriginal patients with palliative care needs in both hospital and community settings, ensuring culturally competent and safe practices in care coordination.

This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a multidisciplinary team environment. The ideal candidate will have a deep understanding of issues affecting Aboriginal people in relation to grief and loss, and be able to lead the development of culturally safe environments for Aboriginal patients and their families.

Key Responsibilities:

  • SUPPORT THE DEVELOPMENT OF CULTURALLY APPROPRIATE END-OF-LIFE CARE STRATEGIES
  • COLLABORATE WITH MULTIDISCIPLINARY TEAMS TO ENSURE RESPECTFUL AND CULTURALLY SAFE PRACTICES
  • ACT AS A CONDUIT FOR ABORIGINAL PATIENTS WITH PALLIATIVE CARE NEEDS
  • LEAD THE DEVELOPMENT OF CULTURALLY SAFE ENVIRONMENTS FOR ABORIGINAL PATIENTS AND THEIR FAMILIES

Required Skills and Qualifications:

  • Aboriginal or Torres Strait Islander descent through parentage, identification, or acceptance within the community
  • Understanding of issues affecting Aboriginal people in relation to grief and loss
  • Ability to lead the development of culturally safe environments for Aboriginal patients and their families
  • Strong communication and interpersonal skills
  • Current NSW Driver's Licence (P1 and P2 acceptable)
