Aboriginal Palliative Care Advocate

We are seeking an experienced End-of-Life Care Specialist to join our team.
The ideal candidate will have a strong understanding of issues affecting Aboriginal people related to grief, loss, social, and emotional wellbeing.
This role involves working collaboratively with clinicians, community-based service providers, and patient families at the primary health care level.
- Acting as a conduit for Aboriginal patients with palliative care needs in both hospital and community settings.
- Ensuring culturally competent and culturally safe practices are integrated into care coordination.
- Leading the development of culturally safe environments for Aboriginal patients and their families within a multidisciplinary team.
Key responsibilities include:
- Developing and implementing culturally sensitive care plans.
- Providing education and training to healthcare professionals on cultural safety.
- Collaborating with local communities to develop and implement culturally safe initiatives.
A current NSW Driver's Licence (P1 and P2 acceptable) is also required.
