Role Overview

The Case Manager plays a vital role within the multi-disciplinary team delivering high-quality participant-centered intake and case management for individuals and families in crisis.

Working alongside other teams, the Case Manager proactively identifies crisis presentations, develops action plans to support housing stability, and facilitates social cohesion through on-site support activities.

Key Responsibilities:
  • Conduct comprehensive assessments of clients' needs and develop individualized service plans.
  • Collaborate with multidisciplinary teams to ensure seamless coordination of services.
  • Develop and implement strategies to enhance housing stability and social cohesion among clients.
Requirements

To be successful in this role, you will require:

  • A Diploma or degree in Social Work, Community Services, or a related field.
  • Experience in homelessness, housing, or crisis casework, particularly in Aboriginal communities in the NT.
  • Strong case management, assessment, and planning skills.
  • Ability to work collaboratively in a diverse multidisciplinary team environment.
