Crisis Accommodation Case Manager

We are seeking a dedicated professional to deliver timely, participant-centered intake and case management for people experiencing crisis accommodation needs at Crerar Road.
This role involves working with the multi-disciplinary team to support housing stability and wellbeing, facilitating referrals, and encouraging participation in site activities.
The ideal candidate will possess strong case management, assessment, and planning skills, as well as effective communication, culturally responsive practice, and advocacy skills.
- Relevant tertiary qualification (Certificate III or above) in social work, community services, welfare, or a related field – OR equivalent experience in crisis, homelessness, or case management support
- Strong ability to work collaboratively in a diverse, multi-disciplinary team
