Meaningful Life Coordinator Job Description

The Meaningful Life Coordinator is responsible for developing, implementing and coordinating meaningful engagement across the care community that will influence and enrich the quality of life for residents. This is delivered through stimulating activities that support holistic wellbeing.

The coordinator will have an understanding of resident's person-centred needs, while partnering with families and the community to provide enjoyable experiences that consider residents' lifestyle preferences.

Key Responsibilities:
  • Develop, implement and coordinate meaningful engagement that include activities reflecting a person-centred model of care, in line with Aged Care Quality Standards.
  • Participate in the coordination of new and/or existing meaningful activities and person-centred engagement that meet residents' goals, needs and preferences.
  • Coordinate and be responsible for all meaningful life residents' assessments, care plans, resident iCare profiles and documentation.
  • Lead and support team members through departmental and organisational changes ensuring clear consultation and communication processes are in place.
Required Skills and Qualifications:
  • Certificate III/IV qualification in dementia care, leisure and health, aged care or leadership or relevant experience and/or willingness to obtain.
  • Excellent collaboration, accountability, innovation, respect and compassion skills.
