About Our Care Community

We are seeking a dedicated professional to join our care team in Ballarat or Edenhope. This is an exciting opportunity to be part of our new model of care, implementing meaningful engagement that gives daily purpose to our residents.

The successful candidate will be responsible for delivering engaging activities and person-centred support, meeting residents' goals, needs, and preferences. Key responsibilities include:

  • Implementing person-centred engagement across the care community, reflecting a model of care in line with Aged Care Quality Standards;
  • Delivering engaging activities and person-centred support that meets residents' goals, needs, and preferences;
  • Completing residents' assessments, care plans, and documentation as directed by the coordinator;
  • Providing support to other team members, volunteers, and students in the care community;
  • Attending regular team meetings and care community meetings as directed by the coordinator;
  • Assisting with organising resources required to support the program;
  • Buidling connections and partnerships within the care community and the wider community;
  • Supporting residents to attend regular scheduled bus outings and connect with the community;
  • Assisting residents and relatives with technology to facilitate communication via scheduled video calls and phone calls;
  • Responding to complaints or concerns raised in a timely manner;
  • Reviewing, documenting, and responding appropriately to potential risks, incidents, and near misses;
  • Maintaining equipment used in leisure and lifestyle programs and reporting any maintenance or damage to the team leader/manager;

Requirements include:

  • A Certificate III in Aged Care (or equivalent) or Certificate IV in Leisure and Health (or equivalent), or willingness to obtain;
  • A qualification in dementia care is desirable;
  • An understanding of Aged Care Quality Standards;
  • Excellent communication skills, with the ability to work as part of an inter-disciplinary team, with strong computer and customer service skills;
  • Experience with aged care software is desirable;
  • A Victorian Driver's Licence;
