Job Summary

This is a Casual Level 1-2 Social Workers role, where you will provide an effective and efficient social work service addressing the psychosocial aspects of care for patients.

Your Key Responsibilities:
  • Deliver intervention including support, education and resources to individuals in their bio-psycho-social situation to assist with health events.
  • Conduct psychosocial assessments and plan interventions as required.
  • Organize and facilitate family meetings and coordinate complex case management plans.
  • Work collaboratively within a team approach, alongside other disciplines, to deliver high-quality patient care.
Required Skills and Qualifications:
  • A Degree in Social Work or equivalent qualification providing eligibility for membership of the AASW.
  • Demonstrated knowledge and understanding of the social work role in the context of healthcare.
  • Ability to complete psychosocial assessments and plan interventions as required.
  • Commitment to working within a collaborative team approach.
