Job Title:

Clinical Care Coordinator

We seek individuals with excellent communication skills, a strong understanding of clinical governance, and experience in working collaboratively as part of a team. A Registered Nurse with current registration with the Australian Health Practitioner Regulation Agency (AHPRA) is essential for this role.

Key Responsibilities:

  1. Provide leadership and guidance to the care team, ensuring that all aspects of resident care are delivered to the highest standard.
  2. Develop and implement care plans tailored to meet the individual needs of each resident.
  3. Collaborate with other departments to ensure seamless delivery of services and care.
  4. Attend handovers and provide after-hours support as required.

Requirements:

  • Registered Nurse with current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Minimum three years' experience in aged care
  • Current NDIS Workers Screening Check
  • Current Covid-19 Vaccinations
