Job DescriptionThe ideal candidate will have a strong background in nursing and experience working in aged care settings. As a key member of our leadership team, you will play a critical role in ensuring the delivery of safe and effective care to our residents.
- Develop and implement individualized care plans that cater to the unique needs and preferences of each resident
- Collaborate with other healthcare professionals to ensure seamless transitions of care and optimize outcomes
- Provide education and support to residents and their families on health-related topics and disease management
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
- Minimum three years' experience working in aged care, preferably in a leadership or coordinator role
- Strong knowledge of the Aged Care Act and relevant industry standards
- Excellent communication and interpersonal skills, with the ability to build rapport with diverse stakeholders
