A Care Coordinator is sought by our organization to contribute to the leadership team. The successful applicant will be involved in ensuring that resources are allocated efficiently in line with our strategic plan.
Main Responsibilities:
- Participate in admission and ongoing care planning processes
- Lead case conferencing processes for residents
- Provide after-hours support as required
Requirements:
- Hold a current nursing registration with the relevant professional body
- Demonstrate at least three years' experience in aged care
- Hold a valid NDIS Workers Screening Check
- Be up-to-date with Covid-19 vaccinations
