Clinical Care Coordinator

As a senior clinical professional, you will play a vital role in advancing the health and wellbeing of our residents. Reporting to the Clinical Care Manager, you will utilize your clinical knowledge and experience to contribute to the improvement of care delivery.

The successful candidate will be responsible for the admission and ongoing care planning process, ensuring that individualized plans are developed and implemented to meet the unique needs of each resident.

Key responsibilities will also include case conferencing, attendance at handover, and providing after-hours support as required.

Requirements:
  • Registered Nurse with current registration from the Australian Health Practitioner Regulation Agency (AHPRA)
  • Minimum three (3) years' experience in aged care
  • Current NDIS Workers Screening Check
  • Current Covid-19 Vaccinations (Highly recommended)
Preferred Skills:
  • Experience in the re-accreditation process and related assessment visits through the Australian Aged Care Quality and Safety Commission (ACQSC)
  • Broad knowledge of the Aged Care Industry, Aged Care Act, 1997 and subordinate principles
  • Strong Knowledge of Aged Care Funding Instrument (ACFI)
  • Excellent computer literacy and competency in electronic care systems
  • Experience in accessing Joanna Briggs "best practice" portal with demonstrable understanding of contemporary nursing practice
  • Experience in Key Performance Indicators and an unambiguous understanding of clinical governance and accountability
