Culturally Sensitive Health Support Worker

This role offers a unique opportunity to contribute to the health and wellbeing of Aboriginal and Torres Strait Islander people in greater Brisbane and Logan.
The successful candidate will be responsible for maintaining accurate health records, communicating and interpreting on behalf of clients and other health workers, and taking part in case management and follow up.
Key Responsibilities:- Maintaining accurate health records for clients.
- Communicating and interpreting on behalf of clients and other health workers, providing culturally sensitive and effective support.
- Taking part in case management and follow up, either independently or with other health care providers, to ensure optimal outcomes for clients.
- To be eligible for this role, you must identify as an Aboriginal or Torres Strait Islander person.
- You will need to have a willingness and ability to undertake a Cert III in Aboriginal and/or Torres Strait Islander Primary Health Care.
- Ability to work independently, under guidance or direction, and within a multi-disciplinary team.
- Previous experience working with culturally diverse people is highly valued.
- Demonstrated ability to meet targets and performance outcomes.
- Able to communicate sensitively and effectively with Aboriginal and Torres Strait Islander People.
- Current and valid National Police Check.
- Working with Children's Card (Blue Card) or be eligible to apply.
- Disability Worker Screening Card (Yellow Card) or be eligible to apply.
