Job Description

The opportunity has arisen for a skilled Enrolled Nurse to join our Home Care team in the Central Coast. As a dedicated professional, you will be responsible for assessing and coordinating care and support services for participants requiring Commonwealth Home Support Services and Home Care Packages.

You will play a pivotal role in providing clinical care, as well as supporting, educating and resourcing consumers to enable informed decisions and exercise choice and control over their services and support.

We are seeking an individual with exceptional communication skills and the ability to collaborate effectively with multidisciplinary teams to deliver high-quality care.


About You
  • Registration as an Enrolled Nurse
  • Sound knowledge and experience working with people who are ageing from an enablement, restorative and wellbeing perspective
  • Understanding of multidisciplinary teamwork and the ability to multi-task
  • Proven ability to conduct comprehensive assessments and develop goal orientated care plans
