Job Title: Clinical Care Specialist

We are seeking a skilled Clinical Care Specialist to provide high-quality patient care in a fast-paced primary care setting.

Key Responsibilities:
  • We will assess, plan, develop, implement, and evaluate programs of care to improve health outcomes.
  • The successful candidate will perform routine nursing tasks, including clinical observations and measurements.
  • The role involves providing holistic care and delivering evidence-based treatment plans.
  • Responsibilities include administering and monitoring medication according to protocols and standards.
  • Enable patients and families to understand their health conditions and develop self-management skills.
  • Prioritize health problems and intervene in emergency situations, initiating effective emergency care as needed.
Requirements:
  • Current professional registration with the relevant governing body is required.
  • Evidence of continuous professional development is essential.
  • Qualification in performance and interpretation of spirometry with relevant certification is necessary.
  • Long-term condition management experience, particularly in Asthma/COPD, is highly valued.
Preferred Skills and Attributes:
  • BLS training is highly desirable.
  • Excellent communication and interpersonal skills are essential for this role.
  • The ability to work autonomously and as part of a team is crucial.
  • Flexibility and adaptability are key attributes for this position.
