Delivering Quality Care Services
We are seeking a dedicated and compassionate professional to join our team as a Grade 2 Social Worker in the General Medicine Unit at Monash Medical Centre, Clayton.
This role involves delivering high-quality care services to patients and their families, while maintaining and building clinical expertise. You will be part of a multidisciplinary team that works together to make a meaningful impact on people's lives.
Key Responsibilities:
- Manage a clinical caseload across designated areas, providing exceptional patient care and support.
- Apply advanced psychosocial assessment and planning skills to guide interventions, ensuring the best possible outcomes for patients.
- Navigate and integrate service systems such as Family Violence, Child Protection, My Aged Care, NDIS, and Mental Health services to provide comprehensive support.
- Participate in quality improvement initiatives and research projects, contributing to the development of best practices in social work.
Qualifications and Requirements:
- Bachelor of Social Work or Master of Social Work Qualifying, or equivalent as assessed and approved by the Australian Association of Social Workers (AASW).
- Eligible for ordinary membership of AASW if overseas trained.
- A current Victorian Driver's Licence.
