About the Role

As a Home Support Specialist with West Wimmera Health Service, you will play a vital role in enabling individuals to maintain and increase their independence.

The successful candidate will provide support services for individuals receiving community-based support, including Traffic Accidents Commission (TAC) clients, National Disability Insurance Scheme (NDIS) participants, and/or community aged care Commonwealth Home Support (CHSP) and Home Care Packages (HCP) clients.

You will assist clients/participants to achieve their individual goals, as outlined in their Support Plan or Care Plan, and promote positive community attitudes in relation to disability, ageing and inclusiveness.

Key Selection Criteria
  • Effective Communication and Organisational Skills: You possess excellent communication skills, both verbal and written, and are able to manage multiple tasks and priorities efficiently.
  • Worker Screening Clearance: You hold a valid NDIS Worker Screening Check, Working With Children Check, and have undergone annual police checks if required.
  • Commitment to Empowerment and Inclusion: You share our commitment to empowering individuals to make informed decisions about their lives and promoting inclusive practices in all aspects of service delivery.
  • Certifications and Licenses: You hold a current First Aid Certificate Level 2 and a Victorian Driver's Licence.
  • Desirable Qualifications and Experience: A qualification in Aged Care, Disability, Individual Support, Community Services, or equivalent field is highly desirable, as is previous experience working in community or residential aged care or disability settings.
Salary and Benefits

We offer a competitive salary of $65,000 - $75,000 per annum, depending on experience, plus a range of benefits, including professional development opportunities and a supportive work environment.
