Job Opportunity: Complex Case Manager

We are seeking a skilled and dedicated individual to join our multidisciplinary team as a Complex Case Manager.

  • The successful applicant will provide evidence-based psychotherapeutic interventions for individuals experiencing alcohol and other drug (AOD) related issues in Frankston and the Mornington Peninsula.
  • They will work within the Reach Out program to deliver AOD interventions and case coordination to patients of the Pharmacotherapy Clinic, Peninsula Health.

A key aspect of this role is working collaboratively with clients, their families, and healthcare professionals to achieve client-centred goals. This includes reducing AOD use, minimizing harm associated with AOD, improving quality of life, and enhancing mental wellbeing.

Essential Criteria:

To be successful in this position, applicants should possess:

  • Tertiary qualifications relevant to the role, such as Social Work, Counselling, or Nursing.
  • Completion of the AOD Skill Set (AOD Core Competencies).
  • Demonstrated experience working with at-risk and vulnerable populations, particularly those with dual diagnosis.
  • Sound knowledge of evidence-based psychotherapeutic interventions for dual diagnosis.
  • Proven ability to manage competing tasks effectively and prioritize accordingly.
  • Dedication to ongoing professional development and evidence-based practice.
  • Well-developed communication, interpersonal, and computer skills.

What You'll Need:

For salary information, please refer to the Allied Health Professionals (Victorian Public Sector) Enterprise Agreement 2021-2025 or Nurses and Midwives (Victorian Public Sector) Enterprise Agreement 2020-2024. Sub-grading will be based on years of experience.
