Adelaide PHN is seeking a skilled professional to coordinate and monitor the Care Finder program.

About the Role

The Capacity Building Coordinator will be responsible for establishing and maintaining a network of Care Finder organisations across Adelaide PHN region.

  • This will involve working closely with key internal personnel and cross-sector organisations to identify local needs and implement solutions;
  • Facilitating multiple contracts within the Care Finder program and identifying opportunities for service improvement and integration;
  • Connecting with others to build relationships and strengthen networks;
  • Meeting data reporting requirements and supporting continuous improvement of the program.
About You

To be successful in this role you'll need:

  • A sound understanding of the health and aged care sectors;
  • Exceptional team player skills with high level interpersonal and communication skills;
  • Ability to engage, partner and collaborate with stakeholders and community groups;
  • Demonstrated experience working in the health sector, preferably in aged care services;
  • Experience researching and critical analysis of complex issues in human service delivery and service development.

Previous experience in roles such as Health Care Professional, Project Manager, Capacity Builder, Service Developer, Clinical Coordinator, or Business Development Service Coordinator may be beneficial.
