
Healthcare Community Specialist

We are seeking passionate and skilled healthcare professionals to join our Community Care Team.
As a Community Health Professional, you will deliver person-centred care to your clients in the community, ensuring a safe environment and applying your clinical skills to improving their quality of life through activities that include:
- Assessing and planning client care within scope

- Creating and updating care plans to reflect developing needs
- Ensuring clinical quality compliance standards are met
- Collaborating with other multi-disciplinary team members
With a diverse caseload and varying environments, you will spend most of your day on the road, completing scheduled community visits.
