We're looking for a dedicated professional to join our team as a Community Health Worker. In this role, you'll serve as a liaison between the community and healthcare systems, providing psychosocial support to individuals with chronic, acute, or terminal illnesses.

Job Description

Serves as a bridge between the community and healthcare providers to provide clients with the necessary resources and support to cope with health issues. Collaborates with other professionals to evaluate patients' medical conditions and assess client needs. Advocates for clients to resolve crises and facilitates access to resources and services to improve service delivery quality and cultural competence.

Required Skills and Qualifications
  • Experience working in health fields such as hospitals, public health, or social service agencies.
  • Completion of an approved Certified Community Health Worker Training Program is preferred.
