
Care Coordinator for Vulnerable Youth

As a dedicated Case Manager, you will be responsible for supporting vulnerable children and young people in Out Of Home Care (OOHC) by developing, implementing, and reviewing client plans to achieve the best possible outcomes.
The successful candidate will possess strong assessment and risk mitigation skills, with the ability to engage effectively with clients and their families. They will also be able to facilitate open communication and coordinate with various service providers to ensure comprehensive support delivery.
Requirements:- A degree in social work, early childhood, psychology, or a related field is essential.
- At least 12 months of case management experience working with at-risk children, young people, and/or families in a child protection-focused role is required.
- Excellent understanding of child protection issues and client vulnerabilities such as drug and alcohol use, domestic violence, mental health issues, and the impact of trauma on child behavior and development is necessary.
- Understanding of the needs of diverse communities including Aboriginal and Torres Strait Islander, culturally and linguistically diverse (CALD), and LGBTIQ+ communities is required.
- Good written and verbal communication skills are essential.
- Current driver's license is necessary.
- Ability to build strong relationships with different stakeholders is crucial.
