Job Description:

We are seeking a dedicated and compassionate Care Manager to join our team. The successful candidate will be responsible for delivering high-quality care to vulnerable young people, working closely with the therapeutic team to develop and implement individualized care plans.

The Care Manager will lead the supervision, support, and development of the therapeutic youth work team, promoting a culture of excellence and continuous improvement. This role requires strong leadership and communication skills, with the ability to work collaboratively with colleagues and stakeholders.

Key Responsibilities:

  • Develop and implement individualized care plans for young people
  • Lead the supervision, support, and development of the therapeutic youth work team
  • Work collaboratively with the therapeutic team to promote a culture of excellence and continuous improvement

Requirements:

To be successful in this role, you will need:

  • Tertiary qualification in social work, community services, or a relevant field
  • Proven experience in developing behavioral management programs for children and adolescents with challenging behaviors
  • Ability to participate collaboratively and constructively within teams of peers, staff, and other stakeholders
